In order to better serve you in a timely manner please bring the following to your visit.
- New patient forms printed and filled out
- Please register for our patient portal and fill out the health history questionnaire. This will integrate with your chart and save you time when you come in for your appointment.
- A complete list of all of your medications and allergies.
- Any relevant tests that pertain to the reason of your visit with us. This includes but is not limited to imaging studies, lab tests or operative reports.
